So you've known for at least two years that you had a billing problem, yet apparently chose to be very nonproactive. You've probably blown any chance you had to appeal the insurer's claim by not acting promptly - but start by getting the detailed billings from the hospital and the insurer's Explanations of Benefit (claim processing forms) - so you can at least find out WHY the claims were supposedly denied. You've got to know the facts before you can fix anything.

Have you contacted the hospital? I would first start with a copy of the bill and they may have a copy of the EOB which explains why it was not covered. The whole situation is nuts. If you call the insurance company back, ask to speak with a supervisor or manager as well.

I would hire a lawyer to get them scared. Insurance companies hate lawyers. I hired one which was my uncle, and he scared the **** out of them...
